You have now completed your course and are ready to test your
knowledge as well as submit your final required assignments and
paperwork to become a Certified Hypnosis practitioner with the
American School of Hypnosis. Below you will find the
Assignment Description
to ensure you have completed both of them and the
Submission Checklist
to submit everything.
CHECKLIST
TO SUBMIT TEST, ASSIGNMENTS AND GRADUATION
REGISTRATION
1.
Create 2
scripts on any titles you prefer that you have not seen already.
(Originals)
If you need guidance in completing them you will find printed guidelines
to help you at the beginning of Chapter 9 (Suggestions - Scripts) in
your manual.
Length of the script is not important as long as they have the
pertinent content necessary to instruct the client to develop the
necessary change desired.
You are encouraged to write about titles you are familiar
with or that you feel you will be needing in your practice. Keep in mind
that new students in future classes will have the opportunity of using
these scripts you created in their practices.
Email your two completed
scripts to
admin@choosehypnosis.com
in either doc,
pdf or in the actual body of your email.
Be sure that the email also has your
name and a short note as to whether you
have already mailed in your Graduation
Packet yet and if not when you are going to do
so. This is important to let us
know as we
process many applicants daily and do not
wish to lose or misfile your
requirements which would only delay the
process.
We will not review your scripts or
paperwork until all your forms have been
mailed and received in the office so as
to save time.
2. Go to

to get your Graduation Packet consisting of 3 Forms and your
Test.. You are to COPY onto Paper
and complete the Forms and Test found there. Then all 3
forms and test are to be mailed to the Home Office. The procedure and
mailing address are all included on those pdf pages
The First one is your Registration Form. However you enter
your name on the form is how it will be printed on your certificates.
The Second is your Assignment Form
(must be signed)
The Third is your AIA Code of Ethics
Agreement Form (must be singed)
and then is the Test
If
for any reason you do not pass
the test you can retake it in a minimum
of 10 days.
NOTE on
PROCESSING: Due
to the large volume of graduates
we
process certificates on the 1st of each month, so if
your paperwork arrives to our PO Box
before then you can get yours sent out
quicker. Be sure you have entered the proper postal address for us to
send them to.
Your Welcome
letter will contain the
url webpage of
your AIA Hub Page containing all of your
additional AIA benefits, manuals and
materials.
Once we have processed your test and
paperwork you will be mailed your
certificates and welcome letter via USPS. You will be notified
once you receive your certificates in
the mail, or in
the event you did not pass we will
contact you via email. Thank you.
Allow approximately 4 - 5 weeks for delivery
from the time we have received them.

"postage due" it will not be
accepted and returned to you due to the

(requiring a signature) if you
want us to receive it. Mailing it
delivery confirmation or just normal
mail (without signature required)
is sufficient.

customer leads and referrals, be sure to
email us with:
* A professional looking picture
of yourself in jpg format. -
This
could be taken from a quick cam, digital
phone or even scanned. We will
crop it to fit the general look of the
other practitioners there. We will
not submit a listing without a picture
due to uniformity.
* The contact phone number and
email address that you would like
clients to contact you at.
* City and state you will be
working out of, business name and
webpage if you have one yet.

LATE FEE:
You are allowed up to one year from your
purchase date to complete this course.
You can extend that an additional year
for a fee of $45.00 USD. Once you
have completed your course, if you have
gone over one year simply include a
check made payable to the American
School of Hypnosis for the $45.00 or
call us directly once you have submitted
your paperwork at (207) 590-5859 to pay
with a credit card over the phone. Any
submission made after one year without
the late fee paid will not be processed.
